Summary This position is eligible for Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Responsibilities The Clinical Pharmacist - Inpatient and Outpatient, is the primary professional pharmacy staff member responsible for the daily operations of the pharmacy medication management systems, to include Ward Stock, Unit-Dose, IV Admixture, non-sterile compounding, sterile compounding, and Ambulatory Care (Outpatient Pharmacy) pharmacy programs. Duties include but are not limited to: Responsible to keep the Supervisory Pharmacist, and other pharmacy staff member's informed of administrative problems or episodes of clinical pertinence which may impact on pharmacy program effectiveness, or which may enhance staff performance. Responsible for the day to-day operational effectiveness of the pharmacy program to which he/or she is assigned. Ensure that all Pharmacy Service and other regulatory policies are complied with in regard to medication labeling, inventory management, formulary management, equipment maintenance and timeliness of service. Maintains and improves professional competency by keeping up with information on new medications, new medication therapies, and innovations in pharmacy practices. Participation in drug use evaluations or other quality assurance activities. Monitoring Inpatient and Outpatient's in all areas of pharmacy practice as an ongoing activity to include: The therapeutic appropriateness of the patient's drug regimen. Therapeutic duplication in the patients' drug regimen. The appropriateness of route and method of administration. The degree of patient compliance with the drug regimen. Clinical and pharmacokinetic laboratory data to evaluate the efficacy of drug therapy and to anticipate side effects, toxicity, or adverse effects. Physical signs and clinical symptoms relevant to the patient's drug therapy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates: (KSAs must be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). GS-11 Pharmacist: Experience, Education, and Licensure: None beyond the basic requirements. GS-12 Clinical Pharmacist: Experience or Education: In addition to the basic requirements, candidates must meet one of the following: 1. One (1) year of experience equivalent to the next lower grade level, or 2. Completion of an ACPE-accredited Pharm.D. program. Demonstrated Knowledge, Skills and Abilities: In addition to the basic requirements, the candidate must demonstrate the following KSAs: 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. However, the work environment requires someone with the ability to handle several tasks at once in sometimes stressful situations.
