Job Summary and Qualifications
The Advanced Clinical Pharmacist is responsible for providing all levels of pharmacy clinical services. This includes providing an advanced level of consultative or direct pharmaceutical care as a member of the health care team in the trauma and/or critical care setting. This includes monitoring patient medication regimens and providing appropriate interventions as well as supporting all aspects of pharmacy operations. The practitioner is expected to collaborate with physicians, nurses, and other health professionals to design, implement, monitor, and evaluate pharmacotherapy to ensure effective, safe, and economical patient care. Advanced Clinical Pharmacist will assist the Director of Pharmacy and Clinical Manager in the development of Pharmacy clinical services and participate in the clinical training of pharmacists, pharmacy residents, students, and other health professionals. Practices according to the policies and procedures of Ocala Health System. May be required to work at Ocala Regional Medical Center, West Marion Community Hospital, or The Free Standing Emergency Departments as needed.
Proficient in all skills as per Clinical Pharmacist essential duties.
Develop, promote, maintain, and evaluate patient care services to achieve appropriate care standards, quality improvement, and cost savings.
Demonstrates competence in the areas of critical thinking, interpersonal relationships, and technical skills.
Review non-formulary requests and provide overall formulary management for the critical care and trauma areas.
Assumes an active role in multi-disciplinary teams and committees such as Pharmacy and Therapeutics, Medication Safety Team, or other clinical task teams or committees as needed.
Attends meeting asrequired
Provide clinical pharmacy services to patients in the ICU and/or Trauma areas.
Assist the Clinical Pharmacy Manager in developing the clinical goals of the Pharmacy department.
Maintain an active role in the Medication Utilization Evaluation and Adverse Drug Reaction reporting.
Supports staff in the Pharmacy department and provides help andassistancewhen possible.
Keeps fellow employe03es in the department informed of relevant information
Establishes andmaintainseffective working relations among the critical care and/or trauma areas.
Disseminate information through intermittent distribution of department, committee or hospital newsletters with critical care information and updates.
Perform other related duties as assigned or requested by the Director of Pharmacy and Clinical Pharmacy Manager.
Interpret orders and transcribes or verifies them accurately and efficiently into the pharmacy computer system
Accurately compound and dispense pharmaceuticals
Complies withfacility policy and procedures and regulatory requirements
Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
Provide clinical consultation and clarification to practitioners. Suggestsappropriate,costeffectivetherapeutic alternatives to medical staff as needed.
Sustain the hospital formulary, minimizing non formulary procurements,utilizingtherapeutic interchanges and promotes rational drug therapyselection
Attend and activelyparticipatein daily multidisciplinary and bedside rounds
Support the hospital formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drugutilizationevaluations
Provideaccurateandtimelymedication information to healthcare staff and patientsActivelyparticipatein pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
Participate in the antimicrobial management and other clinical programs
Document all clinical activities and interventions accurately and completely. Responsible for mentoring and education as well as ongoing analysis of interventions to insure accurate and appropriate documentation and provide summary reports (if assigned)Support and participate in the goals of the organization's quality improvement, risk management and patient safety program to include but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication related initiatives.
Complete notification reportsregardingmedication errors and adverse drug reactions.
Participatein the data collection of drugutilizationevaluations, adverse drug reactions, and pharmacy quality initiatives.
Participate in the training of new pharmacists, pharmacy students, residents, technicians,nursesand physicians.
Participate in preceptorship of pharmacy students and/or residents.
Maintain professional competency sufficient to meet current and future needs of the facility.
Has knowledge of the patient populations served andapplythat knowledge in the care of those patients.
Provide basic technical support for automated dispensing machines and other pharmacy computer systems.
Effectivelysupervisepharmacy operation activities and provides support to the whole pharmacy team.
Compliant with facility pharmacy schedule to work physically at patient care areas to interact with medical staff,nursesand other members of the healthcare team.
Effectively and proactively intervene in conflicts between pharmacy and medical staffregardingdrug therapy issue resolutions.
Train andparticipatewith Disaster Team drills and real-event response as assigned
Train andparticipatewith Code Teams as assigned. Obtain and maintain BLS-ACLS or PALS current certification as appropriate.
Maintainappropriate controlledsubstance records. Work with the Pharmacy Director andappropriate managertoidentifyprocesses to meet or exceed all controlled substance diversion prevention and monitoring standards.
Comply withall regulatory standards. Work with the Pharmacy Director toidentifyand develop processes to meet or exceed all regulatory standards.
Prioritize work and lead others tocomply withfacility medication turn-around time policy and procedure.Identifyareas for improved efficiency.
Supervise andassistpharmacy technicians and other support staff effectivelyParticipate in establishing pharmacy goals andobjectivesandassistin their implementation.
Participate with various nursing and medical staff committees as necessaryAct as supervisor in charge as assigned.
Participate in the continuous quality review of order sets and/or computer drug entries.Participate in the continuous quality improvement of pharmacy operations (operational/clinical).
Provide input for improving pharmacy computer system programs relating to pharmacy,nursingand physician programs.
Perform other duties as assigned.

What qualifications you will need:
4 yearBachelor Degreeor Pharm.D. Required.
FloridaPharmacist License required
BLS required
ACLS required
2 - 5 years of pharmacy experience required
Residency preferred
5+ years of pharmacy experience preferred
