The Regional Case Management Director has the overall responsibility for managing and coordinating case management department activities. The Director ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the market's patient care and discharge planning processes with related market departments and external agencies to ensure continuity of care and optimal clinical resource utilization. The Director has oversight for all aspects of daily market case management operations and is accountable for achieving established outcomes through actively engaging interdisciplinary teams and external stakeholders.

Responsibilities
Lead and direct case management services, workflow and resources.
Formulate and implement operational strategies and initiatives in the case management program.
Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes and issues.
Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.
Requirements
Licensure / Certification / Registration:
Required Credential(s):
One or more of the following:
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) obtained prior to hire date or job transfer date.
Counselor obtained prior to hire date or job transfer date.
Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date.
Social Worker credentialed from the Texas State Board of Social Worker Examiners.
Respiratory Care Practitioner obtained prior to hire date or job transfer date.
Respiratory Therapist credentialed from the National Board for Respiratory Care (NBRC) obtainedprior to hire date or job transfer date.
Social Worker obtained prior to hire date or job transfer date.
Education:
High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required. Master's degree preferred.
Work Experience:
5 years of experience required.
10 years of experience preferred.
2 years of leadership or management experience required.
5 years of leadership or management experience preferred.
Additional Preferences
RN License, LCSW or LMSW License required.
2+ years' experience in case management leadership, preferred.
3+ years' experience in overall acute care Hospital Case Management, preferred.
Master's degree in Nursing, Health Administration, or Business Administration, preferred.
