Overview

Provides psychosocial services to patients, families and caregivers facing the impact of a cancer diagnosis. They are knowledgeable about community and government resources and are able to access for the patient/family. They help facilitate patients' adjustment to cancer. They help patients navigate the

Responsibilities

· Coordinates patient care by serving as the point-of-contact for ongoing psychosocial resources for patients and families throughout the care continuum pre-diagnosis through treatments into survivorship.

· Communicates pertinent information regarding patient issues with physicians and other members of the care team.

· Collaborates with support services (palliative care, hospice, dietician, psychologist, chaplain, etc.) regarding how to best to meet psychosocial needs.

· Purposely conducts all aspects of the job in an ethical manner, consistent with the Social Work Code of Ethics and in support of the hospital's commitment to ethical behavior in all areas of personal and professional activity.

· Demonstrates a level of understanding of oncology diagnosis and treatment modalities and how the patient is affected.

· Works in collaboration with the care team to develop and monitor clinical quality indicators that reflect the entire continuum of care.

· Assists in the assessment, development, and implementation of programs that enhance the psychosocial services offered to cancer patients and their families.

· Assists patients in understanding their diagnosis, treatment options and the resources available, including educating eligible patients about appropriate clinical research studies.

· Assists in referral of cancer patients for cancer risk assessment, genetic counseling and testing services.

· Coordinates psychosocial screening.

· Assesses social, emotional and economic needs of the patients and their families in relation to the impact of illness.

· Identifies patients with compromised coping skills or lack of social support.

· Provides counseling regarding support services options and appropriate referrals.

· Assists patients with financial concerns and basic needs which impact their health.

· Acts as an advocate for patients with cancer and for the Cancer Center program.

· Works effectively with other caregivers to develop optimal systems, which achieve the most effective patient/family outcomes.

· Coordinate's survivorship care with the oncology team.

· Provides continuity of care by ensuring smooth transitions between care settings

· Develops a relationship with patient and their primary care physician to facilitate and/or navigate through a cancer diagnosis, subsequent treatment and follow-up to reflect continuity of care.

· Develops patient education programs and tools. Design and select patient education materials on specific disease sites.

Qualifications

Education:

Bachelor's degree in social work, psychology or related field.

Experience:

Two (2) years of social work, or related work and healthcare experience preferred.

License(s)/Certification(s):

Licensure appropriate to degree Basic Life Support, refer to job Description Addendum.

Knowledge/Skills/Abilities:

Effective written/verbal communication skills. Knowledge and experience in good interviewing techniques. Ability to understand medical terminology and communicates effectively with persons of diverse backgrounds, age, disabilities, and education levels required. Able to demonstrate knowledge, skills and abilities to provide services to Oncology patients and families.
