Adhere to the professional standards of CPH, the American Physical Therapy Association and NYS Office of Professions. Actively supports and participates in patient coverage efforts across all departments as necessary. We proudly offer outpatient health services in Brasher Falls, Canton, Colton, Gouverneur, Massena, Norfolk, and Potsdam, NY.STATUS: Per DiemLOCATION: Lawrence Ave, PotsdamDEPARTMENT: Physical TherapySCHEDULE: 7.5 Hour shiftATTRIBUTESRequired: Master's Degree in Physical Therapy as of 8/12 Preferred: DPT, or PhD P.T. RESPONSIBILITIESCompletes physical therapy evaluations to establish treatment plans and home programs to restore function, relieve pain & prevent disability. Adheres to all hospital and departmental policies and procedures. Administers patient treatment plan involving application of physical therapy techniques and modalities as indicated. Monitors response and progress to update and modify treatment plan as necessary to meet patient goals. Instructs patients and family members / caregivers in physical therapy procedures and self-care techniques. Directs care that is provided by the physical therapy assistant by providing oversight. Performs discharge planning and documentation to provide discharge instructions and maintain post-discharge continuity of care. Consults with physicians and other health care professionals regarding treatments, progress, patient needs, and outcomes. Participates in self and others' education, training and development, as applicablePerforms any other related duties as assigned. Additional documentation that is not performed during evaluation and treatment sessions. PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.PAY RANGE: $39.00 - $52.00 The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
