Position Summary
Under the supervision of the Chief Health Officer, the Nurse
Practitioner provides primary health care services as a member of a
health care team comprised of Health Partnership Clinic (HPC)
physicians, advanced practice providers and clinical support staff.
HPC employs a team-based approach to delivering patient care. Clinical
staff work with their team to effectively use huddles at the start of
each clinic day, utilize standing orders when applicable, use
evidence-based approaches to patient care such as teaching
self-management to patients, emphasize seamless care coordination for
all patients, strive to understand the vulnerable populations served,
and adjust services to fit patient needs. Upon hire, all staff receive
orientation regarding the benefits of the HPC team-based approach to
population health.
Duties and Responsibilities:
Perform health assessments, including history and physical examination,
order and/or perform diagnostic tests, evaluate pertinent psychosocial
and family history, perform general health assessments, and diagnose
acute and chronic conditions.
Promote family health, including periodic health assessments and
screening for early diagnosis.
Assess and guide families about home safety, childhood hazards, optimal
health practices and provide mental and emotional resources as needed.
Manage common acute and chronic illness, including minor trauma,
episodic problems, common chronic disorders, as well as counseling and
education with regard to such problems, performance of minor surgical
procedures and initiation of life-saving procedures in emergencies.
Supervise staff (if assigned), including making recommendations for
promotion, commendation, or disciplinary action where necessary and
appropriate.
Refer complex and high priority cases to an HPC physician, regarding
complicated diagnostic problems, serious illness, complicated
therapeutic problems, or re-evaluation of chronic conditions.
Work as a member of the health care team and assist in the education of
patients and the community in preventive health care and health
maintenance concepts.
Demonstrate patterns of continued learning to ensure growth as a
practitioner.
Exhibit awareness of community resources (insurance products, social
services, etc.) that may help improve the overall health and well-being
of patients.
Flexibility and adaptability.
Clinic office hours are generally 7:30 am to 5 pm, Monday Friday, with
intermittent Saturday duties. Hours are subject to change as needed.
Other duties or responsibilities as assigned.
Regular attendance and punctuality are essential requirements of this
job.
Qualifications
Must be licensed/certified as a Family Medicine or Adult Medicine
Nurse Practitioner; must have active, unrestricted KS license as a
Nurse Practitioner.
Maintain an active and unrestricted KS DEA number.
Current CPR (BLS) certification.
Must be eligible for liability coverage via FTCA and private malpractice
insurance carrier.
Approval of Kansas Charitable Care Provider Application.
Staff membership and privileges granted by the HPC Board of Directors
(approval of credentialing and privileging applications).
Ability to relate well with patients and other health care personnel.
Ability to maintain appropriate clinical privileges.
Social and cultural sensitivity appropriate to ethnically and
economically diverse patient and employee base.
Ability to work in a fast-paced environment.
Commitment to expanding access to care for the underserved.
Ability to work with a multi-disciplinary team of medical professionals,
Behavioral Health Clinicians and Dentists.
Ability to communicate fluently in Spanish, preferred.
Knowledge of public health principles and practices, preferred
Ability to work with Electronic Health Record System.
