JOB PURPOSE:
The Home Health Consultant collaborates with the Home Health Agencies by evaluating and educating agencies/facility staff for compliance with all regulatory groups by making recommendations and suggestions for improvement. Track, trends and reports data needed to monitor compliance and agency standing in relation to national benchmarks, outcomes, policy implementation and other @types of agency performance standards.
KEY RESPONSIBILITIES:
Knowledge of Home Health Care operations and federal and state regulations.
Must demonstrate effective interpersonal skills to interact with agency/facility staff, corporate leaders and others.
Must have a working knowledge of PI principles and techniques.
Must know methods to comply with the regulations.
Conducts mock surveys of assigned locations annually and as needed. Mock surveys may include research and documentation of medical record information, personnel files, committee reports, policies and procedures, and company manuals to assure compliance and conformance with state and federal regulations as well as other @types of regulatory groups such as Joint Commission

Supports Home Health agencies in achieving high quality outcomes and patient satisfaction scores.

Keeps supervisor informed of daily activities, and issues. Proposes solutions to all issues.
Review and analyze the adverse outcome reports.
Monitor and analyze information from Home Compare Indicators.
Assists with development of plan of correction to survey deficiencies.
Reviewing additional information requests from Medicare and Medicaid as requested.
Assists Operations with coordinating communication and correspondence with The Joint Commission.
Disseminates appropriate information regarding agency statistics, tracking and trending to supervisor.
Attend seminars and workshops related to Home Health Care to increase own knowledge and to keep current the fields of Medicare/Medicaid/OSHA state rules and regulations, The Joint Commission and other @types of regulations that may apply.
Performs focus review audits on specific topics of interest quarterly to facilitate improvement in clinical outcomes as well as providing an education opportunity for the Agency.
Interprets and translates new agency policies and procedures and assesses compliance with implementation of new policies and procedures.
Develops and participates in educational programs throughout the company related to Home Health Care compliance and other clinical issues as assigned.
Maintains absolute confidentiality of agency/facility records including, but not limited to payroll, personnel, patient/client care, business plans, and computer program.
Participates in policy development and implementation.
Assists to provide orientation to key positions on the working of the Clinical Teams, how policy and procedures are produced, etc.
Collaborates with the agencies toward meeting the annual clinical objectives.
Responsible for bringing forward issues of concern affecting quality of care in the agencies.
KNOWLEDGE, SKILLS, ABILITIES:
Participate in center/agency surveys (Licensure/ The Joint Commission) and any subsequently required reports and/ or plans of correction.

Complies with corporate compliance program.
Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary.

Follows established safety procedures when performing tasks and/or working with equipment.
Performs other related duties as necessary and as directed by supervisor.
Attends and participates in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required.
Attends and participates in mandatory in-services.
Possesses Knowledge of State and Federal regulations governing post-acute & long-term care.
Clinical practice expertise - program scope and implementation.
Training and Education - ability to formulate and deliver educational programs.
Basic computer software skills: Word, Excel, PowerPoint, Internet/Intranet use.
Exceptional communication skills with peers, professional staff, consultants and patients in
interdisciplinary care setting and to government agencies.
Knowledge of emergency and disaster preparedness.
Exceptional knowledge of quality assurance and performance improvement and data analytics.
MINIMUM EDUCATION REQUIRED:
· Must be a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist in the states where the company operates.
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Must have at least 5 years in administrative supervisory roles in home health care.
Ability to travel (up to 80%) and work independently.
