Responsibilities
Job Summary:
Provides hands-on direct patient care including evaluation, treatment, education, discharge planning and documentation per department standards as a member of the healthcare treatment plan. This would include coordinating and modifying patient's care, treatment goals and discharge planning recommendations to achieve a maximum level of patient functional independence. Position also provides expertise in areas of manual therapy, lymphedema, hand therapy, informatics, sports medicine, emergency care or neurologic rehabilitation.
Essential Functions:
Interviews patient, family, caregivers, significant others to obtain information about prior level of
function, home environment, family relationships, health and psychosocial history.
Performs client tests, measurements and evaluations as indicated for the patient's condition and as per
any established best practice guidelines and/or department care standards.
Establishes measurable/functional treatment goals that are realistic and consistent with
patient/family/caregiver needs.
Plans and prepares written treatment programs based on evaluation, age-related and diagnosis-
related policies. Administers treatments as prescribed in the treatment plan. Evaluates effectiveness of
treatment and adjusts the treatment to achieve maximum benefits.
Monitors all patients under his/her care: supervises Physical Therapist Assistants and/or Technicians
who are involved with treatment, and assures appropriate treatment and documentation; submits a
modified plan of care to the physician for altered plans of care; reports any adverse reactions to
treatment or changes in condition to the physician as indicated.
Institutes or assists with emergency measures for sudden adverse developments such as cardiac arrest or medical conditions requiring activation of a code, rapid response team, or activation of 911.
Implements and assesses appropriate patient and caregiver education per Plan of Care and phases of
progress.
Establishes and maintains collaborative communications with physicians, other healthcare
professionals, and facility departments. This would include team conferences, CCU rounds, other
patient care rounds or assigned meetings.
Consistently meets department productivity standards (90%) for outpatient and (75%) for inpatient.
Completes required reports, notes and other client-oriented documentation in a timely manner
according to policy and department standards, including G-code functional reporting and associated
modifiers.
Complies with changing regulations related to insurance, The Joint Commission, state/federal and other requirements as directed.
Documents medical necessity and skilled need for continued treatments in evaluations/re-evaluations
and progress notes.
Meets facility standard for Focus on Therapeutic Outcomes (FOTO) status and discharge surveys
(minimum of 10 completed per quarter). Uses FOTO recommendations appropriately or documents
reasons for exceptions (i.e. progress to date and expectation of significant progress if patient extended
beyond FOTO recommendations). (outpatient)
Enters charges accurately into Electronic Health Record or on appropriate charge slip.
Consistently participates with department communication processes: checking email daily, attending at least 80% of all scheduled meetings and in-services, and reviews meeting minutes if unable to attend.
Maintains specialized training requirements for a certification in Lymphadema (LANA), hand therapy
(CHT), manual therapy (FAAMOPT), Acute Care, Neurologic rehabilitation (APTA NCS or GCS and
specialty training in PNF, constraint induced movement therapy, vestibular stimulation or facilitation
therapies), emergency care or informatics (documented training in current EMR, FOTO, Meditech and
Redoc).
Provides public or other health care provider or staff training per year.
Participates in developing approaches for specialized services in area of expertise, in coordination with physician office to educate the community of specialty rehabilitation services.
Other duties as assigned by management.

Qualifications
Required Education and Experience:
Doctoral degree in Physical Therapy from an APTA accredited school of Physical Therapy
Required Licensure and Certifications:
Current license issued by the State of Oregon as a Physical Therapist or current PT compact privilege to practice in Oregon
AHA BLS certification within thirty (30) days of hire
Required Minimum Knowledge, Skills and Abilities :
Incumbents in this position must maintain a current knowledge of safety issues needed in the treatment of patients, especially regarding body substance precautions and body mechanics to protect self, patients, and personnel working with therapists.
The incumbent must abide by the ethical guidelines as outlined by the American Physical Therapy Association.
Ability to prioritize, organize, and use time management skills, problem solve, and productive communication.
Must possess a high degree of precision and accuracy.
Good written and oral English communication skills; excellent interpersonal skills.

Pay Range
$42.49 - $61.61 /hour
