Case Manager RN
Description:
Minimum Qualifications: RN with a minimum of two years of experience in nursing or other clinical area; previous experience in Case Management/Utilization Review preferred. This position calls for effective interpersonal skills to interact effectively with all levels of hospital personnel as well as organization and prioritization skills. Proficient computer skills, effective written and verbal communication skills are also necessary.
Primary Responsibilities include but are not limited to: Case Management, Swing Bed and Performance Improvement. Function as a liaison between patients, medical staff, administration, nursing, finance and other departments to promote cost effective inpatient utilization. Review patients for Swing Bed and communicate with UR Director and physicians when patient appropriate for Swing Bed admission. Work with the Quality Improvement team to assist with meeting quality measures and participate in committee meetings as requested by supervisor.
