Changing the age-old story starts with you - let's get you hired! Service Area: North Central (coverage area: Golden Valley, Robbinsdale, Brooklyn Center, Brooklyn Park, Anoka, Coon Rapids, Oak Grove & surrounding). Plus mileage reimbursement of .70 cents per mile. Schedule: Full Time - Monday through Friday 8am to 5pm, Weekend rotation every 10 weeks. Wage Range: $91,000-$110,000 annual salary, depending on experience. The Physical Therapist is primarily responsible to plan, organize, direct homecare services. Experience in physical therapy with emphasis on community health education experience. The professional physical therapist builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Job Duties: The Home Health Physical Therapist is primarily responsible for delivering high-quality physical therapy services in the patient's home, following the standards of care and adhering to all conditions outlined in the Service Agreement. Key Responsibilities Include: Any direct client care provided will be performed as defined by the state Physical Therapy Practice Act. Working with patients to improve patient outcomes related to mobility, strength, balance, and function to help them regain independence at home. Monitoring patients' progress and adjusting treatment plans as needed to ensure optimal results. Collaborating with the healthcare team to provide the best outcomes for the patient. Managing and sharing clients with PTA team. Qualifications: Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association. Current and active Physical Therapist license in the state of Minnesota.
