Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Responsibilities This Licensed Practical Nurse (LPN) position is in Primary Care services at the Boston Healthcare Systems, located at the Jamaica Plain location. The Licensed Practical Nurse (LPN) works under the direction of a RN and reports to the Primary Care Nurse Manager. The LPN is a member of the Patient Aligned Care Team (PACT). He/she demonstrates active collaboration and communication with the Teamlet (Patient, Primary Care Provider, RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient). Works in collaboration and partnership with the teamlet for an assigned panel of patients to promote effective prevention or chronic disease management. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. He/she provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The LPN participates in the collaboration with services internal and external to the VA to facilitate care transitions that effectively meet the patients' needs.
