Actalent is Hiring a Team of Remote PAC Nurses!Job Description
The PAC Nurse is a telephonic position responsible for recommending discharge plans, assisting with the transition of care, and managing the length of stay (LOS) for Long Term Acute Hospital (LTACH), Skilled Nursing Facility (SNF), and Institutional Rehab Facility (IRF) patients. This role involves collaboration with internal and external stakeholders to ensure timely and effective discharge planning.
Responsibilities
Collaborate with key facility personnel and internal Medical Directors, Market Engagement Directors, and Nurse Managers to develop and maintain timely discharge plans.
Work with the PAC Medical Director to ensure proper medical necessity decisions are being made.
Partner closely with the PAC Medical Director in care planning, goal setting, reviewing discharge plans, and monitoring length of stay status.
Act as a clinical resource for unlicensed Post-Acute Care Coordinators, providing clinical expertise and clarifying referral source directives.
Respond to requests from unlicensed staff regarding scripted clinical questions and issues.
Serve as the primary contact for assigned post-acute facilities, obtaining all required clinical information and patient status updates.
Work alongside the Supervisor and collaborate with Market Engagement Directors to address potential facility concerns, pushback, or gaps in the process.
Communicate customer service or provider issues to the supervisor for logging and resolution.
Conduct scheduled telephonic touch points with each facility point person to review each member within that facility and confirm appropriateness for continued stay.
Authorize continued stay at SNF, IRF, LTACH, and Home Health care (if delegated) using approved medical care guidelines and collaboration with key facility personnel.
Use clinical expertise to review clinical information and criteria to determine medical necessity for services/devices.
Ensure case reviews and determinations are completed within contractual and regulatory turnaround time standards.
Participate in performance and operational improvement activities, quality assessment/improvement activities, and ensure data collection for improvement analysis.
Assist in implementing and maintaining standardized operational processes to ensure compliance with company policies, legal requirements, and regulatory mandates.
Participate in special projects and other assigned duties.
Participate in annual Inter-rater reliability Testing Process.
Maintain a typical work schedule and provide evening and weekend coverage as needed.
Essential Skills
Current and unrestricted LPN or RN license
Post-acute nursing experience in Inpatient Rehab Facility, Long-Term Acute Care Hospital, or Skilled Nursing Facility
3 years of concurrent review experience and/or discharge planning
2 years of utilization review/management experience
1 year of experience in Case Management or Transition-of-care role
Additional Skills & Qualifications
Comfortable speaking with providers and offices via phone
Experience with Interqual and Milliman/MCG
Knowledge of URAC & NCQA standards
Broad knowledge of health care delivery and managed care regulations
High-level clinical knowledge, communication, customer service, and problem-solving skills
Strong organizational and time management skills
Work Environment
This is a fully remote position with equipment provided. The initial training period lasts three weeks, during which attendance is mandatory. Training schedule: Monday-Friday, 8:00am-4:40pm EST. Post-training schedule: Monday-Friday, 11:00am-7:30pm EST (30-minute lunch break). Candidates from PST and MST are welcome to apply to better align with the schedule. After a 90-day probationary period, candidates are eligible for up to 40 hours of PTO and 6 paid holidays. There is an opportunity to transition to a permanent role with a large managed care organization.
Pay and Benefits
The pay range for this position is $32.00 - $39.00/hr.
