Physical Therapist
Job Description
We are seeking a dedicated and compassionate Physical Therapist to evaluate patients, determine treatment plans, and communicate with the care team. This role involves preparing documentation such as evaluations, daily notes, progress notes, and discharge summaries. The ideal candidate will consult with the rehabilitation team to select treatment programs, develop home programs to enhance patient performance, and create treatment protocols using evidence-based guidelines and patient-specific goals.
Responsibilities
Evaluate patients and determine treatment plans.
Prepare documentation including evaluations, daily notes, progress notes, and discharge summaries.
Consult with the rehabilitation team to select treatment programs and coordinate services.
Develop home programs to enhance patient performance.
Create treatment protocols using evidence-based guidelines and patient-specific goals.
Initiate and respond to referrals from healthcare professionals.
Maintain a positive service-oriented approach with patients, families, caregivers, and staff.
Monitor patient progress and adjust treatment plans as needed.
Train and evaluate therapists, and assist with orientation of new staff.
Manage equipment maintenance and supply inventory.
Commit to reducing costs and improving productivity.
Handle changing patient schedules and maintain positive relationships with the healthcare team.
Assist with Quality Assurance reports and facility reports.
Participate in program development, department meetings, and comply with policies.
Promote company philosophy and ensure quality care delivery.

BS, MS, or DPT degree in Physical Therapy.
Current and valid Iowa PT license.
Current Healthcare Provider CPR certification required.
Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire).
Ability to work independently and exercise sound decision-making.
Ability to accept and work with diverse populations, culturally, economically, and educationally.
Must have a valid driver's license, auto liability insurance, and reliable transportation if driving is required.
Work Environment
You will work out of our Creston, Iowa location, providing care to patients in homes and nursing facilities in the surrounding area, primarily working with elderly populations. Travel is required using your own vehicle between locations and homes. A valid driver's license, auto insurance, and reliable transportation are imperative, with mileage reimbursement provided. Physical requirements include exerting up to 50 pounds of force occasionally, up to 20 pounds frequently, and up to 10 pounds constantly. Physical activities include climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, talking, typing, hearing, grasping, repetitive motions, feeling, and visual acuity. You may be exposed to extreme temperatures, hazards such as chemicals and infectious diseases, and atmospheric conditions like fumes, odors, dusts, mists, and gases. Typical hours are 7:30 AM - 4 PM with flexibility for varying schedules and programmatic requirements, with the latest appointments typically until 5 PM.
Pay and Benefits
The pay range for this position is $95517.00 - $114620.00/yr.
