Registered Nurses provide comprehensive nursing care to patients across the age spectrum and with acute conditions. Registered Nurses apply established professional nursing concepts, principles and practices to assess, plan, implement and evaluate nursing care for general medical and/or surgical patients. Responsibilities Duties by Departments/Specialties: Nurse Case Managers Serves as the team leader of the interdisciplinary healthcare team promoting the coordination, collaboration, and continuity of care from preadmission to post-discharge from a variety of health care providers (doctors, nurses, nurse practitioners, specialty and contract providers, nurse midwives, physical therapists, dieticians, pharmacists, medical social workers, and mental health practitioners) Incumbent ensures patients receive quality patient centered care that is consistent with the patient-centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to achieve desired clinical outcomes. Nurse Clinical Care Coordinator Prioritizes the information collected based on the patient's condition and preferences, the situation, and/or anticipated needs. Develops a plan of care with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s). Leads the team in panel management activities to ensure all patients receive appropriate preventative and chronic disease management Nurse Emergency Room Determines the severity of illness or injury and initiates and provides rapid intervention in emergent, life-threatening conditions, even when a physician is not present or immediately available. Administers therapeutic measures as prescribed by the physician to including, but not limited to medication administration, intravenous fluids, blood transfusions and other emergency treatments. Facilitates Emergency Department Patient flow and efficiency to expedite medical evaluation and treatment. Nurse Intensive Care Unit Provides comprehensive nursing care to patients by utilizing the nursing process to assess the needs of patients, prioritizing and organizing the plan of care from admission through discharge. Collaborate with other health care professionals to identify individual patient needs and to develop, implement and evaluate plans of patient care for critically ill patients. Administers therapeutic measures as prescribed by the physician to include, but not limited to, monitoring life functions, administering prescribed medication, and monitoring for compliance. Nurse Post Anesthesia Care Unit Delivers comprehensive nursing care specific to the age of the patient (adult, geriatric, pediatric, neonate) undergoing a wide variety of surgical procedures, ranging from simple to complex in nature, under both controlled and stressful situations-including on-call hours. Administers therapeutic measures as prescribed by the physician to include, but not limited to, medication administration, intravenous fluids, and blood transfusions. Independently manages patients receiving sedation/analgesia/anesthesia to include anatomy, physiology, pharmacological modalities, cardiac arrhythmia recognition, and complications related to adverse effects of procedural sedation and general anesthesia. Nurse Medical/Surgical Utilizes the nursing process to identify individual patient needs and to develop, implement and evaluate plans of patient care for acutely ill patients. Provides professional skilled, competent nursing care specific to age of the patient (adult, geriatric, obstetric, pediatric, neonate) to meet physical, emotional, spiritual and social-cultural needs of the patient, by means of direct care and teaching. Observes and assesses patient's clinical condition: recognizes, identifies and interprets serious situations and immediately implements proper action. Nurse Community Health Formulates and implements overall community health nursing programs and plans to satisfy varying patient load requirements, priorities for service and other healthcare objectives. Tracks community health status and evaluate health trends and risk factors of population groups. Collects data in a systemic organized manner and analyzes the collected assessment of data. Nurse Outpatient Assesses patient's needs and health status data, individually and/or with the care team to develop a plan of care. Accepts responsibilities as team leader and/or charge nurse. Coordinates clinical and patient flow, prepares assignments and provides guidance and leadership to nursing personnel of varied skill levels. In the absence of the supervisor, accepts those supervisory and administrative responsibilities. Nurse Inpatient Performs professional nursing care of medical, surgical, orthopedic and pediatric patients. Performs administrative and clinical duties. Administers therapeutic measures such as medications, intravenous fluids, blood transfusions, etc. Nurse OB Assesses patient care needs through physical, psychosocial, educational and discharge needs specific to pregnancy, laboring, post-partum OB/GYN, surgical and newborn patients and determines priorities of care based on assessment.
