About the Role

We're a Florida-licensed, ACHC-accredited skilled home health agency hiring a full-time, W-2 Physical Therapist to join our clinical team. This is a salaried position with predictable pay, benefits, and the back-office support to keep documentation manageable.

You'll work autonomously in the field with a clustered caseload, backed by an interdisciplinary team that actually communicates.

What You'll Do

Perform skilled PT evaluations, develop individualized plans of care, and deliver treatment in patients' homes

Set functional, measurable goals that move patients toward independence

Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility

Complete timely, accurate visit documentation in our EMR

Coordinate with RNs, OTs, and ordering physicians as part of the care team

Communicate status changes promptly so the team can respond

What You Bring

Active Florida PT license in good standing

Current BLS/CPR certification (we run an in-house AHA training site if you need to renew)

Reliable transportation, valid FL driver's license, and auto insurance

Strong clinical assessment, documentation, and communication skills

Comfort working independently in the field

Home health experience preferred strong new grads welcome to apply

Compensation and Benefits

Salary: $100,000 to $105,000 annually, W-2

Productivity standard: 30 visits per week

ETO: Paid time off accrual from day one

Mileage reimbursement for patient travel

Geographic clustering we route visits to minimize windshield time
