Responsibilities:

Develop, implement, and maintain person-centered Community Habilitation service plans based on individual goals, needs, and preferences.

Coordinate initial and ongoing assessments to identify strengths, needs, and skill-building opportunities.

Ensure that services are delivered in accordance with the individuals Life Plan, habilitation plan, and regulatory requirements.

Train, support, and provide guidance to Direct Support Professionals (DSPs) assigned to Community Habilitation cases.

Ensure staff understand habilitation goals, teaching strategies, safety protocols, and documentation expectations.

Maintain accurate, timely, and comprehensive documentation, including progress notes, service logs, data collection, workflow trackers and monthly summaries.

Track individual progress toward community habilitation goals and adjust plans as needed.

Serve as the primary point of contact for individuals, families/guardians, support teams, and external providers.

Attend care team meetings, Life Plan meetings, and interdisciplinary conferences as needed.

Ensure all services adhere to OPWDD (or applicable state agency) regulations and HCBS waiver requirements.

Participate in internal audits, quality improvement initiatives, and trainings mandated by the agency.

Qualifications:

Bachelors degree in Human Services, Psychology, Education, Social Work, or a related field (preferred).

Experience working with individuals with intellectual and developmental disabilities (IDD).

Knowledge of HCBS waiver services and person-centered planning.

Strong organizational, communication, and problem-solving skills.

Ability to work independently, manage multiple cases, and adapt to individual needs.

Valid drivers license and reliable transportation (not a requirement but preferred).

Ability to pass background checks and agency-required trainings (e.g., SCIP, CPR/First Aid).

Schedule: Monday to Friday

Salary: $65,000/Yr.
