About Us

Lake Forest Adult Day Health Center is an Adult Day Health Care (ADHC) center currently under development in Lake Forest, California. The center is expected to serve approximately 75 older adult participants and provide nursing, rehabilitation, social services, nutrition services, and therapeutic activity programs through an interdisciplinary care team.

Position Summary

The Physical Therapist will provide participant assessment, treatment planning, therapeutic exercise, mobility training, fall prevention support, and consultation to the interdisciplinary ADHC team.

Qualifications and Requirements

Current and active California Physical Therapist license required. Must be in good professional standing and legally authorized to provide physical therapy services in California. Must be able to evaluate participant physical function, mobility, balance, strength, endurance, and safety needs. Must be able to develop and document treatment plans and progress toward care plan goals. Experience in geriatric rehabilitation, fall prevention, neurological conditions, or community-based care preferred. Current CPR/BLS certification preferred or required before start date.

Preferred Qualifications

Bilingual English/Mandarin preferred. Experience with older adults and ADHC participants preferred.

Key Responsibilities

Conduct physical therapy evaluations and reassessments. Develop individualized treatment recommendations and care plan goals. Provide therapeutic exercise, mobility, transfer, gait, and balance training as appropriate. Educate participants, families, and staff on safety and fall prevention. Participate in interdisciplinary team meetings and progress review. Maintain timely treatment records and progress documentation.
