Descriptive Title:Nurse Supervisor

Full-time, Monday to Friday (with some alternate evenings), 9:00am - 5:00pm, 37.5 hours per week.

Description of Duties:

This is a grant funded position. -Serve as team leader and role model for professional and paraprofessional nursing staff; ensure development, execution and adherence of SHC nursing policies and procedures. -Supervise and coordinate issues around nursing team work flow, including development of schedules for cross-coverageand task completion, lunch breaks, and any relevant late or swing shifts. -Participate in the development and implementation of teaching plans for patients, family, and/or caregivers and promote patient self-care activities and treatments that facilitate optimal levels of functioning. -Provide direct patient care involving: nursing care assessments (both in person and over the phone); participation in the evaluation and screening for patients receiving buprenorphine; administration and oversight of medications and vaccinations; and assist with procedures such as pap smears. -Provide telephone and walk-in triage for acute patient problems and make appropriate referrals. -Participate in the development and implementation of multi-disciplinary case managed care, including around long-acting injectable medications. -Document all plans and treatment in the electronic health record, and complete documentation for NYS DOH and other regulatory agencies. -Participate in quality improvement activities, including data tracking and development of PDSA cycles. -Collaborate with outreach team on outreach activities, including provision of HIV testing and education. -Assist in coordinating services for new and emerging SHC programs, and assist with administration responsibilities as needed.

Qualifications:

REQUIRED: Registered nurse with NYS license; BSN from accredited nursing school with at least 3 years of ambulatory care experience. Proficient in the use of Microsoft Office, Electronic Health Record, and hospital-based applications.. PREFERRED: Master's degree in Nursing, public health or related field. Experience in the care of HIV/HCV and substance use disorders. Experience in academic medical center with integrated care/research. Experience with regulatory requirements of Patient Centered Medical Home (PCMH).
