We are currently seeking a licensed and motivated Physical Therapist to provide rehabilitative services to patients in the comfort of their homes.

Position Summary: The Physical Therapist (PT) will evaluate and treat patients with physical impairments or disabilities, focusing on improving mobility, restoring function, and reducing pain. This role involves working independently in patients' homes, developing customized therapy plans, and collaborating with interdisciplinary teams.

Key Responsibilities:

Conduct in-home evaluations and develop individualized care plans

Provide therapeutic interventions to improve strength, balance, and mobility

Educate patients and caregivers on exercises and use of adaptive equipment

Monitor patient progress and adjust treatment plans as needed

Collaborate with physicians, nurses, and other care team members

Accurately document all visits and maintain compliance with state and federal regulations

Requirements:

Active Physical Therapist license in the state of Indiana

Graduate of an accredited Physical Therapy program

At least 1 year of clinical PT experience (home health preferred)

Valid driver's license and reliable transportation

Strong communication, time management, and clinical skills

Ability to work independently and manage a flexible schedule
