EXPECTATION / REQUIREMENTS

Complete start of care assessments only

Primary focus on patient care

Accurate, timely documentation

Conflict resolution skills

Critical thinking skills

Motivated self-starter with an approachable, friendly, and positive attitude

OASIS Assessments

PDGM -- Must have worked in this newest payment model

Professional license -- Current and unrestricted

Strong communication, organizational, and time management skills

Willingness to learn and help support other team members

Monthly virtual team meetings

Ability to attend 2-3 mandatory in-service/skills labs in person per year

Reimbursement is provided for time and travel
