The direct care worker job description consists of the following duties, tasks, and responsibilities:

Assist clients with activities of daily living like bathing, dressing, grooming, and using the toilet

Prepare simple meals and snacks according to client preference and/or special dietary needs

Provide light housekeeping duties, such as cleaning living areas, laundry and changing linens

Monitor vital signs of clients, such as blood pressure and temperature, and report issues to healthcare professionals

Assist clients in transfer and ambulation techniques, including bed to wheelchair transfers

Administer medication under the guidance of a nurse or healthcare provider, as permitted within state law

Provide companionship to clients and offer emotional support

Involve and assist clients with exercise and physical activities as appropriate

Maintain proper documentation about clients' care, activities, and progress

Communicate with clients, family members, and health professionals about all relevant information

Provide recreation and socialization activities for clients

Follow client care plans; carry out the instructions of nurses or other supervisors

Encourage client independence and self-sufficiency whenever possible

Assist with scheduling, and facilitating, medical appointments, including transportation to and from the appointment, if necessary

Manage client finances and budgets

Provide transportation for the client for appointments, etc.

Keep the client safe and prevent falls or other accidents.

Maintain confidentiality and privacy for clients.
