We are seeking a compassionate, skilled, and dedicated Registered Nurse (RN) to provide high-quality home health services to patients in their homes. The ideal candidate will be committed to delivering patient-centered care, collaborating with interdisciplinary teams, and ensuring compliance with all applicable regulations and agency standards.

Position Summary

The Registered Nurse is responsible for assessing patient needs, developing and implementing individualized care plans, providing skilled nursing services, educating patients and families, and supervising clinical care provided by professional and auxiliary personnel. This role requires strong clinical judgment, excellent communication skills, and a commitment to enhancing patient outcomes while promoting independence and quality of life.

Key Duties and Responsibilities

Patient Care & Clinical Services

Perform initial patient assessments and ongoing reassessments, addressing physical, mental, emotional, cognitive, social, and spiritual needs.

Develop, implement, supervise, and update individualized patient service plans.

Provide nursing care and services as ordered by the physician or healthcare provider.

Educate patients and families regarding diagnoses, medications, diet, activity levels, disease management, safety practices, and infection control.

Monitor patient condition and communicate changes in status to appropriate staff and providers.

Respect and protect patient rights, including confidentiality, privacy, and the right to refuse services.

Care Coordination & Supervision

Direct and supervise professional and auxiliary personnel providing patient care services in accordance with Pennsylvania regulations, the Pennsylvania Nurse Practice Act, and agency policies.

Coordinate care with physicians, healthcare professionals, departments, disciplines, and community resources.

Collaborate with management and clinical leadership to ensure quality patient care and service delivery.

Assist with staffing coordination and support adequate coverage of patient care needs.

Documentation & Compliance

Maintain accurate, complete, and timely clinical documentation in accordance with agency requirements and regulatory standards.

Comply with all applicable federal, state, and local laws, regulations, and agency policies.

Maintain an active Pennsylvania Registered Nurse license and required certifications.

Professional Development & Leadership

Participate in staff orientation, in-service programs, and continuing education activities.

Attend agency meetings, patient conferences, training sessions, and educational programs.

Assist with organizational improvement initiatives and program development.

Maintain awareness of industry trends and contribute professional recommendations to improve services and operations.

Represent the agency professionally within the community and when interacting with patients, families, referral sources, and the public.

Qualifications

Required

Associate Degree in Nursing (ADN) from an accredited nursing program.

Active Registered Nurse license in the Commonwealth of Pennsylvania.

Current Basic Life Support (BLS) certification.

Ability to pass state and federal criminal background checks.

Strong communication skills in English, both written and verbal.

Ability to work independently and make sound clinical decisions.

Positive attitude, professionalism, and commitment to patient-centered care.

Preferred

Bachelor of Science in Nursing (BSN).

Previous home health, community health, or case management experience.

Physical Requirements & Working Conditions

Provide patient care in home-based settings.

Ability to sit, stand, walk, bend, stoop, and perform patient care activities for extended periods.

Assist with lifting, repositioning patients, and moving equipment or furniture when necessary.

Travel to patient homes and agency meetings using a personal vehicle.

Occasional travel to conferences, seminars, or training programs may be required.
