Non-Medical Care Manager

AFW2 Program ensures transition services are provided to WII Airmen/Recovering Service Members (RSMs). The goals of care, management and transition reform includes establishing a process for integrated continuity of world-class quality care and service delivery. The process focuses on quality assurance and improved resource allocation of medical and non- medical resources and services throughout the continuum of care from recovery and rehabilitation to reintegration. It serves as a collaborative process of inter-service, interagency, intergovernmental, public and private collaboration through unified common standards of care, standards of practice, comprehensive recovery plan and national resource directory.

Non-Medical Care Manager (NMCM) assists WII Airmen, families, and caregivers to navigate through the continuum of care. The NMCM has the primary responsibility for the management and support for the Recovering Service Member's (RSM's) and their families care management, throughout the range of care from recovery, rehabilitation, and reintegration in active duty, civilian life, and as a veteran. The CM will work with a multi-disciplinary team to support the RSMs and their families for the development and management of the Comprehensive Recovery Plan (CRP).

Non-Medical Care Manager

Job Duties:

Provide specialty support for transition, finance, personnel, and other specific issues to support Airmen.

Oversees the development, implementation and retains oversight of the Comprehensive Recovery Plan (CRP)

Consults and collaborates with Multidisciplinary Teams (MDT) throughout the continuum of care

Oversees, coordinates, and monitors the non-medical services across the continuum of care as documented in the CRP.

Assists in the rehabilitation and transition back to serving reserve status or civilian life as a veteran through coordination with Military Service Coordinators and VA Liaisons.

Collects, maintains, and produces data for reporting purposes in accordance with governing program guidance

Required Skills/Qualifications:

Prior Air Force experience required.

Organization and communication skills.

Interpersonal skills: ability to work with multiple agencies to accomplish project objectives.

MS Office Proficiency and experience with Database Systems.

Must be a U.S. Citizen. Must be able to pass an extensive background investigation.

Required Education Level:

Bachelor's degree, with a background working with military organizations and/or wounded warrior recovery programs.

Additional Skills/Qualifications Desired:

NMCMs must have strong analytical, communication, organizational, and interpersonal skills and can work effectively across multiple agencies.

Previous active duty military experience or experience working with Service members or veterans preferred.

Leadership Experience working with Wounded, Ill or Injured Service Members and/or their families.
