::: {offset-key="8qght-0-0"} Provides nursing services through person-centered planning practices that include medication monitoring, assessment, planning, implementation, and evaluation of individuals at risk or those experiencing psychiatric/physical symptoms. Services will vary with medical orders. :::

::: {offset-key="bar6s-0-0"} Works effectively with Residential Home Managers and other members of the treatment team in a collaborative approach by actively participating in the Home staff meetings to ensure effective communication. :::

::: {offset-key="doie5-0-0"} Demonstrates the ability to educate individuals and staff related to health and medications using current information and best practice guidelines when applicable. Maintains current knowledge by completing required continuing education hours and by seeking educational opportunities to expand knowledge base. :::

::: {offset-key="ccfin-0-0"} Focuses on integrated care and assists the Residential Home Manager with coordination of health care services for individuals within the residential system while attending all medically necessary appointments. :::
