What You'll Do:

Provide physical therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department Procedures.

Conduct screenings of residents at regular intervals to determine need for intervention/treatment.

Evaluate residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated.

Interpret and communicate evaluation findings to residents, families and team members.

Establish individual resident schedules in conjunction with other team members

Develop treatment plans by identifying a target list, including short term and long-term goals and methods to achieve identified goals.

Coordinate treatment plan with resident, family and other team members.

Implement and conduct treatment as outlined in treatment plan.

Monitor resident's response to intervention and modify treatment as indicated to attain goals in accordance with physician orders.

Develop appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.

Qualifications

Current Washington Physical Therapist license

Current BLS Card

Strong clinical skills and a compassionate approach

Dependable and efficient with excellent teamwork

Previous long-term care or rehab experience a plus
