Direct Support Professional

Direct Support Professionals are directly supervised by the Program Manager. All CSW's work flexible hours. The hours worked are determined by the needs of the individuals served.

Direct Service & Support:

1)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Participate in the development of each person's Individual Service Plan, as assigned/needed.

2)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Implement the activities, behavioral and teaching interventions as well as the support strategies that are specified in each individual's service plan.

3)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Assist and support individual's in a range of activities including, but no limited to personal care, daily living activities, and participation in community activities.

4)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Schedule medical appointments as needed. Transport individuals to scheduled medical appointments. Assist in the administration of prescribed medications by each person's physician and complete relevant documentation.

5)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Assist individuals in the preparation of nutritious meals in accordance with their diets to their abilities and wishes.

6)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Assist individuals to spend their personal funds consistent with their needs, preferences and desires.

7)[ ]{times="" new="" roman";="" display:="" inline-block"=""}As needed and desired, arrange for transportation to work, recreation, medical appointments, and other activities.

8)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Ensure individuals have opportunities to exercise their rights and responsibilities.

9)[ ]{times="" new="" roman";="" display:="" inline-block"=""}Through individual and team efforts, promote a lifestyle, which is valued and desired by the individuals living in the home.
