The RN Hospice Case Manager will assist patients in various settings, including patients' homes, hospice facilities, and healthcare institutions. The role involves frequent travel to visit patients and requires the ability to manage physical demands associated with patient care and maintaining scheduling.

This position plays a critical role in ensuring that patients receive high-quality, compassionate care during their final stages of life, making it both a challenging and rewarding career path.

Key Responsibilities

Patient Care Coordination: The RN Case Manager serves as a member of the interdisciplinary team (IDT), coordinating care according to the individualized Plan of Care (POC). This includes assessing patient needs, developing care plans, and ensuring that care aligns with the patient's wishes and regulatory standards.

Patient Education: Educate patients and their caregivers about hospice philosophy, disease progression, and end-of-life care techniques. This includes providing information on managing symptoms and addressing emotional and spiritual needs.

Documentation: Maintain accurate and timely documentation of patient care activities, progress notes, and significant changes in condition, ensuring compliance with hospice policies and regulatory requirements.

Interdisciplinary Collaboration: Actively participate in IDT meetings, preparing in advance and adjusting care plans based on team discussions and recommendations. Communicate effectively with physicians and other healthcare providers regarding patient needs and changes in condition.

On-Call Coverage: Provide on-call coverage as scheduled, responding to patient needs and emergencies as they arise.

Qualifications

Education: A degree in Nursing is required

Licensure: Current, unrestricted RN license in the state of practice is mandatory.

Experience: Previous experience in hospice, ICU/CCU or similar or a related field is preferred, but training can be arranged for the right candidate

Strong emphasis on compassionate communication and crisis management skills.

Skills: Strong critical thinking, time management, and assessment skills are essential. Proficiency in medication administration and knowledge of disease processes are also important.

Bilingual (Spanish) is a plus!
