*Mercy Health Physicians Lorain, LLC, a part of Bon Secours Mercy Health, Inc., has an opening in Lorain, OH for a Family Medicine Physician.

The position of Family Medicine Physician will provide direct patient care at the physician office by managing and coordinating patient care throughout treatment; diagnose, treat and provide medical services to patients at the Medical Practice; demonstrate leadership of the primary care offices' transformation, recognition and operation as a Patient Centered Medical Home (PCMH) which includes team-based care with physicians and staff functioning as the top of their license/training use of EHR for chronic disease management, preventative care, population management and care coordination, and an overall commitment to advancing future models of primary care; attend Physician office councils meetings; prepare and maintain proper medical records and reports in accordance with Mercy Health's policies, standards and requirements and federal regulations; collaborate with and assist administration in; (i) setting operational policies and procedures for medical practice; (ii) recruiting other physicians for hospital and /or medical practice; (iii) establishing and implementing promotional activities, but not limited to, marketing the medical and surgical services, hospital and the medical practice to the community; provide collaboration for mid-level providers as reasonably requested by Mercy Health; assist administration on a daily basis in the administrative and management of the medical practice, including, helping supervise professional and non-professional staff; comply with and participate in quality improvement and utilization review programs of Mercy Health and the Medical Practice; and participate in one or more Physician and/or Administrative committees in order to share and receive information and experiences relating to office practices. This physician will not supervise any staff members. The physician will only oversee the medical care of the patients.

[Schedule:]{.underline} 40 hours/week. On-call schedule: Rotation Approx. 1 every 20 weeks. Weekdays 5pm-7am and weekends 24hrs/day.

[Requirements:]{.underline} MD or DO (or foreign equivalent); Graduate of accredited Family Medicine or Internal Medicine Residency Program; Unrestricted Ohio Medical License; and Unrestricted DEA registration.
