A Skilled Home Health RN provides individualized, physician-directed medical care to patients in their homes, helping them manage chronic or acute conditions, recover from illness or surgery, and maintain independence in a familiar environment. The RN serves as the primary clinical liaison between the patient, family, and the broader healthcare team.

Key Duties and Responsibilities

Patient Assessment - Conduct comprehensive initial and ongoing in-home assessments of patients' health status, functional ability, and environmental safety

Medication and Treatment Administration - Administer prescribed medications and treatments, including IV therapies, injections, and complex wound care

Vital Signs Monitoring - Monitor, record, and report vital signs and any changes in the patient's condition to the supervising physician and care team

Care Plan Development - Develop, implement, and regularly revise individualized nursing care plans in collaboration with the patient, family, and interdisciplinary team

Patient and Family Education - Educate patients and caregivers on disease processes, medication management, self-care techniques, and disease prevention

Care Coordination - Collaborate with physicians, physical/occupational therapists, social workers, and other healthcare professionals to ensure integrated, holistic care

Equipment Management - Manage and maintain in-home medical equipment, ensuring proper use and patient safety

Documentation - Complete and submit all required clinical documentation and records accurately and in a timely manner

Emotional Support - Provide emotional support and counseling to patients and families throughout the care process

Safety Assessment - Evaluate whether the home environment is safe and conducive to recovery; escalate to higher levels of care when needed
