Job Summary

The Medical Records Clerk maintains the policies and procedures established for the record keeping of the nursing facility to comply with all regulations, both State, Federal and Health Information Management Standards. The Medical Record Clerk is responsible for assembling, performing the concurrent reviews as outlined by company policy, storing and protecting the clinical records and maintaining an organized record system for both active and discharged files for prompt retrieval Medical Records staff is responsible to the Director of Nursing and/or Assistant Director of Nursing.

Responsibilities

The Medical Record Clerk/Coordinator works with the Director of Nursing to assure the following responsibilities are accomplished. This list is only partial and should not be interpreted as a comprehensive inventory of all duties, tasks and responsibilities.

Should be knowledgeable about release of information and releasing information according to requests and State and Federal guidelines.

Prepares copies of records when required with assistance of administrator.

Destroys clinical records according to facility policy and procedure and State Statue.

Maintains filing system in an organized manner for both active and discharged records.

Promptly assembles discharge residents for closing in proper Order of Final Filing.

Performs audits to assure completeness of discharge records.

When necessary, prepares the Discharge Summary, outlines the data and obtains all final/transfer/death diagnoses.

Assigns the necessary codes for diagnoses.

Prepares and maintains pre-admission packets and chart "set-ups" for new admissions.

Assembles chart in proper order of Active Chart Order.

Prepares photo for MARs.

Inputs data into computer on all admissions.

Verifies timely physician visits for all residents as required.

Verifies Medicare residents certifications signed timely.

Inputs or audits physician orders daily.

Prints monthly physician's orders.

Thins active records per retention guidelines.

Maintains lab tracking.

Other duties as assigned by supervisor.

Reports immediately to supervisor and administrator situations involving abuse, neglect, or suspicion of a crime.
