Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team!

Schedule: Monday - Friday 8:30am-4:30pm

Duties & Responsibilities:

Provide assessment of newly referred patients and assess health needs in home care setting

Work with patient to create a patient centered plan of care

Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family

Prepare written documentation and summaries of the patient's progress as needed for the continuation of home health services

Provide supervision and training of PCA/HHA staff

Assist with the on-call rotation (stipend provided)

Qualifications:

NYS Licensed Registered Professional Nurse

Minimum of Associate's Degree required

One year Nursing experience required

Home care experience a plus
