Title: RN Case Manager Location: Remote Duration: 12 months Pay Range: $38-$40/hr Shift: 8am - 5pm EST; Mon-Fri

Summary: The Case Manager RN is responsible for leading a multidisciplinary care team to deliver holistic, patient-centered care management for health plan members with diverse medical and social needs. This role serves as the primary point of contact for members, caregivers, and providers, ensuring coordinated care across the continuum.

Key Responsibilities:

Coordinate and lead a team including social workers, dietitians, pharmacists, and providers.

Develop, implement, and monitor individualized care plans using the case management process.

Assess medical, psychosocial, and support needs; identify gaps and barriers to care.

Connect members with community, behavioral health, financial, and medical resources.

Educate members on health management, medications, and nutrition.

Support care transitions (e.g., hospital discharge) and ensure continuity of care.

Advocate for members and promote self-management.

Maintain accurate documentation and meet quality standards.

Required Qualifications:

Active Michigan state RN license

Certification in Case Management (CCM) (required or to be obtained within 18 months of hire)

Health Plan Case Management experience (required)

3+ years of clinical nursing experience

1+ year of case management experience (managed care preferred)

ADN/Diploma in Nursing (BSN preferred)

Preferred:

Experience with telephonic/digital patient management

CCP certification

Motivational interviewing and empathy

Time management and multitasking

Proficiency in MS Office and digital tools
