Advocate/Case Manager reports to Executive Director - Melissa Schmitz

Basic Function:

The Advocate/Case Manager ensures the highest quality of care and services for program clients in a community-involved agency. Case Managers are designed to provide a comprehensive range of services, particularly to vulnerable populations with the most complex needs.

Job Seekers with felony convictions will be considered on a case-by-case basis.

Scope of Role:

The ideal candidate will provide Case Manager services to clients within the scope of the organization and be creative, innovative, and open to change.

Essential Duties and Responsibilities:

Assessing clients' physical and mental wellness, needs, preferences, and abilities, and using these to develop tailored care plans.

Listening to clients' concerns and intervening as required.

Provide case management to the assigned caseload.

Develop and monitor individual service plans in conjunction with agency clients.

Assist agency clients in accessing necessary community services (medical, psychological, substance abuse, vocational, food, clothing, etc.) through the development of a referral network and effective referral and follow-up processes.

Assist agency clients with problem-solving and crisis intervention.

Act as advocate for the consumer as appropriate; encourage and support self-advocacy.

Identification of any problems in daily living skills; hands-on assistance and/or referral in addressing issues in daily living skills.

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.

Handle case assignments, draft service plans, review case progress, and determine case closure.

Assist clients in achieving wellness and autonomy.

Facilitate multiple care aspects, including case coordination and information sharing.

Help clients make informed decisions by acting as their advocate regarding their overall status and resource options.

Develop effective working relations and cooperate with other organizations throughout the entire case management process.

Arrange abeyances with social services, health, and governmental agencies.

Take the extra mile and interact with clients to monitor their progress and ensure it is on track.

Record case information, complete all necessary forms, and produce statistical reports.

Promote quality and cost-effective interventions and outcomes.

Assess and address motivational, biopsychosocial, and other relevant issues.

Adhere to professional standards as outlined by protocols, rules, and regulations.

Ability to utilize the case management system, Charity Tracker.

Attend community events and develop programs tailored to community needs.
