As an LPN, you will provide health screening, coordination of health referrals, and work in close collaboration with prescribers and staff in the Adult and Child/Family Clinics, PROS and Crisis programs under the direction of a Registered Nurse or other authorized health care provider.

Work Schedule: Monday through Friday - 7:30 to 3pm

What your day might look like:

Dispenses medication as prescribed by the Psychiatrist/Nurse Practitioner

Complete weekly checks in the Medication Administration Records

Provides crisis intervention services as needed, including offsite in the community

Forwards reports, labs, notes to PCPs as requested

Facilitate Health and Wellness groups

Conducts urine drug screenings in accordance with agency policies

Utilizes electronic health record system for all patient documentation

Ensures the security and inventory of all medications and nursing supplies

Attends department meetings and trainings as required

Performs other duties as assigned within @context of job description

Your skills and qualifications:

NYS Department of Education Licensed Practical Nurse

Excellent engagement, organizational and communication skills Detail-oriented

Ability to work with electronic database management systems
