As a Registered Nurse (RN) Case Manager, you will play a key role in delivering skilled nursing and supportive care for patient's, guided by each patient's individualized plan of care. Key Responsibilities

Provide skilled nursing care and hospice support in home settings based on the interdisciplinary plan of care.

Perform comprehensive nursing assessments and adjust care plans as needed.

Monitor and manage symptoms, with an emphasis on pain and comfort.

Communicate with patients, families, physicians, and team members to ensure coordinated, high-quality care.

Offer emotional support and education to patients and caregivers about the disease process, medications, and hospice philosophy.

Maintain accurate clinical documentation and update patient care plans regularly.

Supervise and document the performance of home health aides at least every 14 days.

Participate in on-call rotation and respond promptly to patient/family needs.

Collaborate with interdisciplinary team members and attend care conferences.

Qualifications

Current Registered Nurse (RN) license in the State of Michigan.

Minimum 2 years of nursing experience; hospice, oncology, acute care, or home health experience preferred.

Strong assessment and critical thinking skills with the ability to respond to patient needs in a variety of home settings.

Compassionate communication and teaching skills to support families and caregivers.

Comfortable with electronic documentation and data entry.

Valid driver's license and reliable transportation.
